For recipients of Hospice services, a Social Worker completes a comprehensive assessment and care plan in order to provide quality services to patients and their family members. Hospice Social Workers assist patients and family with the following:
Assistance with practical and financial concerns
Social/emotional support
Facilitate communication between patients, family, community agencies
Bereavement follow-up
Hospice Social Work Intervention is comprehensive and support is on-going for the entire duration which Hospice services are provided. Social Work services may also continue after Hospice services are discontinued as a bereavement support for survivors/loved ones.
Hospice Social Workers collaborate with outside organizations like the Dream Foundation fulfilling patient's finial wishes.
